Financial Policy

For your convenience, we accept Visa, MasterCard, Discover, American Express,Cash and Check. We deliver the finest care at the most reasonable cost to our patients, therefore, as a result of the amount of time that we invest in your treatment, along with material and overhead cost, full payment is expected at the time of service unless other arrangements have been made in advance.


This facility is not a provider of any insurance carrier expect for United Concordia Elite PPO. As a courtesy to our patients, we will be happy to complete and forward insurance forms via electronic claim, and we will do so without charge. We do not file claims for Medical Insurance, Medical, HMO, DHMO and Medicare. Please remember you are fully responsible for all fees charged by this office regardless of your insurance coverage. For out-of-Network PPO provider reimbursement from your insurance company will be sent to the patient only. Most insurance companies will respond within one to four weeks. Not all the services we provide are cover benefits. Benefits differ from one Insurance Company to another. You must provide us with an insurance card and all the information necessary to file your claim. Your insurance policy is a contract between you, your employer and the insurance company. We are NOT a party to the contract. Our relationship is with you and not your insurance company.
